Please provide your insurance information through the initial form at least 2–3 days before your appointment. You must also email us a copy of your insurance card so we can verify your eligibility.

Please note that once an appointment is scheduled, that time is reserved exclusively for you. For this reason, we maintain a 48-hour rescheduling and cancellation policy, with no exceptions.

For patients without insurance (self-pay), standard rates are:

Initial Psychiatric Evaluation – $375;

Follow-Up Visits – $170.

Self -pay rates apply only when insurance is not used. The choice of self-pay or insurance-billing must be made prior to the visit and cannot be changed afterward.
